Appointments for existing patients and GP referrals: by email or phone, Subject: New Appointment, include your full name and date of birth and email address (if phoning).
Scripts: email (or phone), Subject: Script, include your full name, date of birth, medicine required, doctor’s name and the name of the pharmacy you use.
